We bridge the gaps in care for clients with SUD by providing health evaluations and care plan management, which increases client engagement, reduces recidivism, and decreases client drug use. All you have to do is submit a referral that takes 1-2 minutes… no more hour-long referral processes! Plus, we engage Medicaid, so our program comes at no cost to your team or your clients, even after probation ends. And—we can potentially pay for drug tests.
Once clients enroll, they get access to telehealth appointments, prescriptions, an SFT/CBT digital therapeutic app with cash rewards, and so much more.
