Holon Health is a multi-specialty provider (including primary care and psychiatry) that bridges the gaps in care for people with substance use disorder (SUD) by providing health evaluations and care plan management. Our services combine face-to-face telehealth with technology, using a proprietary digital therapeutic app to increase patient engagement. We offer varying levels of support from full spectrum care to assisting traditional treatment.

Knowing that 90% of clients involved in the criminal justice system have Medicaid and 70% have SUD, we build relationships with communities and court systems to engage this high-cost population. Our focus on wellness and preventative care improves patient health literacy and the overall patient experience while aiming to reduce relapse and increase periods of sobriety and abstinence. Holon Health reduces healthcare costs by engaging an underserved and hard-to-reach population in preventive care. We offer timely assessments, care planning and coordination, motivational support, and prescriptions (including MAT) via telehealth.
